InsightHostKJ> type a ? if you have a question and wait until I call on you to ask it

sandrich>i just got vioxx today.. saw on t.v. where people had died from it..... what do i do?

DrWaters> No, as long as MD is using common sense about concommittant med's and propper monitoring.

sandrich>sorry

InsightHostKJ> ok...go ahead with the vioxx ? DR waters

lilybell> ?

sandrich> is vioxx dangerous?

DrWaters> A zillion people are on Vioxx and except for edema and inc BP side effects are very rare. Still, it's not my 1st choice.

InsightHostKJ> whats your 1st choice for NSAID?

Harriette> ?

DrWaters> Mobic or Celebrex.

InsightHostKJ> ok lily go ahead

lilybell> rhuemmy put me on minocin, nizoral and metronizole..says will cure my ra..could that be true?

DrWaters> Except for the Minocin never heard of that protocol.

Sue22>?

InsightHostKJ> Swimmer go ahead

swimmer> OK...ENBREL....what about the shortage?

swimmer> Is it temporary?

swimmer> should we consider other meds

DrWaters> Yes, new factory in RI to be done this summer.

InsightHostKJ> will that solve the shortage?

swimmer> what about next week when I don't have meds?

DrWaters> I've changed a lot of patients to remicade or kineret.

DrWaters> RI plant will solve shortage.

InsightHostKJ> so either switch or hang in there?

Lucy-onlooker> ?

lilybell> ?

DrWaters> It's the old would you rather fight or switch.

InsightHostKJ> what about an IM cortisone shot to help boost someone until they get the enbrel?

DrWaters> How about PO cortisone?

InsightHostKJ> what if you're already on po?

DonnaM> ?

td> I am

sassynlexnzac> oh my

InsightHostKJ> LOL I love it

InsightHostKJ> OK harriette go ahead

Harriette> What do you think of the new findings of Celebrex, helping cancer?

DrWaters> It's iffy, but still, it's good news.

sassynlexnzac> ?

InsightHostKJ> Sue22 go ahead

su-boo> ?

Sue22> How do you feel about the new med. Bextra?

Sue22> Vioxx raised my bp.

DrWaters> It seems very good so far except for cost. The only dose approved for regular use is 10mg, not the 20mg.

InsightHostKJ> Lucy, your turn

Lucy-onlooker> I wanted to know what you think about Prosorba treatments. I have never heard of someone on it, but am curious if it would be good for me

Sue22> I am on 10mg...so no more than 20mg?

InsightHostKJ> Lucy i was on it

su-boo> ?

DrWaters> Ask KJ, she's the expert on that. I have NEVER been able to get anyone's insurance to cover it.

Lucy-onlooker> hmmmm ok thanks

InsightHostKJ> Just gotta be a b*tch DR W

DrWaters> I am!

InsightHostKJ> Donna go ahead

DonnaM> Immunex has signed a contract(today) with Genetech to start producing Enbrel in 2003 and 2004 with an option for 205

Lucy-onlooker> lol

InsightHostKJ> LOL! not enough!

sassynlexnzac> lol kj

Lucy-onlooker> well kj I would like to discuss when u have time

Lucy-onlooker> thanks

su-boo> how do u ask??

DrWaters> It's all semi-irelevant because it sounds as if the new ones will be better than Enbrel.

InsightHostKJ> no problem lucy....i'll stick around after chat if you want

InsightHostKJ> su boo wait until i call on you

Lucy> K

su-boo> ok

InsightHostKJ> Sassy go ahead

sassynlexnzac> i usually do need one of those i'm running out of transplants lol

su-boo> I am taking solganol, last resort DMARD for me.. How effective is this in the long run..

Lucy> amen

DrWaters> When it works it can be very effective.

sandrich> ?

DrWaters>su-boo> how long?

su-boo> thank you

DrWaters> orever and then some.

InsightHostKJ> Mel...go ahead

Melanie> I take mtx. Can you have skin reaction to mtx? No NSAIDs at this time.

DrWaters> Not common but definitely yes. Strangely, it often casues rash on tops of hands.

InsightHostKJ> can it cause raised spots on the hands?

Melanie>Mine is on neck. Just wondered. Thanks, Dr. W.

InsightHostKJ> Ron your turn

DrWaters> Yes, raised spots and on neck.

InsightHostRon> My RD has told me that because i have had reoccurence of Histo after enbrel that many of the new drugs including remicade and kineret are not allowed. I have tried about all of the others to no avail. Any thoughts?

Texas> ?

DrWaters> That's probably true unless there's some PO antifungal you can take at same time.

InsightHostRon>

thanks

InsightHostKJ> Texas go ahead

Texas> Italian Docs have article on net concerning use of Rifamycin mixed with joint fluids.. Any positive thoughts about this type of serum?

Katie> ?

DrWaters> Haven't heard about it.

InsightHostKJ> Katie...go ahead

Katie> Can you please explain why some doctors will give intramuscular injections of cortisone during a flare and others are totally against this. My RD say no...internist says yes and that it would help my allergies too.

Texas> www.aicareuma.orgif you have time to read about it Dr Waters..many thanks

DrWaters> Because it's no better than pills and much more expensive.

DrWaters> OK, Texas.

InsightHostKJ> TD go ahead

td> Should I be on anything like calcium supplement when taking 7.5 mg prednisone?

DrWaters> Almost always yes. 600 bid pc

lilybell> not very forthcoming

lilybell> sorry

DrWaters> Fosamax or Actonel for post-child bearing age females and all adult males.

lilybell> what are those?

DrWaters> NSAID's in general don't help much for RA or PA. 0

InsightHostKJ> to prevent osteoporosis in those on prednisone lily

lilybell> never heard mention of those

lilybell> not on pred 3

InsightHostKJ> Sand...did u have another question?

DrWaters> Lilybell needs doctor transplant, too.

InsightHostKJ> that was in answer to someone elses question lily

sandrich> what do you think of glucosamine, shark oil, etc.?

lilybell> amen

Bren> ?

DrWaters> Glucosamine probably is helpful in OA. I tried it but it made me too gassy and I was grossing out too many patients.

InsightHostKJ> ewwww

sassynlexnzac> lol sorry couldnt help but laugh

sandrich> lol

Lucy> hehe

InsightHostKJ> Marcia go ahead

Lucy> just open windows

InsightHostKJ> Stinky Dr!

DrWaters> Doubtful if any supplements other than glucosamine effective.

Katie> not even omega #

Katie> 3

DrWaters> They're sealed shut in Florida.

Marcia> I have been on pred 4 over 13 years, 20mg per day. Just started on the kineret. Do u think if the kineret kicks in, i will b able 2 come off the pred, mayb ever?

DrWaters> Oh yeah, Omega 3 is good too.

Katie> ?

DrWaters> Doubtful.

InsightHostKJ> why?

DrWaters> 20 mg! Maybe get down to 10, but off?

InsightHostKJ> what if she was on 5?

DrWaters> THEN yes.

InsightHostKJ> Sassy your turn

* InsightHostKJ breathes a huge sigh of relief

Melanie> lol

sassynlexnzac> i heard u say that some drugs affect calcium what drugs should i stay away from if i can't produce my own blood calcium? 0

sassynlexnzac> (hypoparathyroidism)

DrWaters> You really need to discuss this with your endocrinologist.

sassynlexnzac> k

InsightHostKJ> Bren go ahead

Bren> You said new drugs will be better than Enbrel. Were you referring to Remicade and Kineret? Any other drugs?

DrWaters> No, I meant the new Abott drug (once every 2 weeks sq) and others not out yet.

Bren> What's the name of it?

InsightHostKJ> Katie go ahead

DrWaters> Don't know.

Katie> Is Omega 3 specifcally good for RA and if so How much per day?

DrWaters> Good for everything but bleeding problems. 3 Grams daily minimum.

InsightHostKJ> D2E7

InsightHostKJ> is the name

Bren> Thanks KJ

DrWaters> Of Star Wars guy?

InsightHostKJ> LOL

InsightHostKJ> Sounds like it

Katie> thanks

Texas> 2d

Katie> R2D2

InsightHostKJ> Dr waters how is kineret working for your patients?

DrWaters> Bad.

Katie> ?

InsightHostKJ> no results?

InsightHostKJ> or side effects?

DrWaters> What's been your experience.

DrWaters> No results.

InsightHostKJ> Best thing that ever happened to me

InsightHostKJ> Katie..go ahead

DrWaters> I'm getting around only 20%% of Enbrel or Remicade failures that respond to kineret. Higher in patients never on those.

Katie> everything on this new drug from Abbot sounds so good...is there a possibility that MTX may be a drug of the past...

DrWaters> Maybe.

swimmer> ?

Katie> oh m y gosh

InsightHostKJ> swimmer go ahead

DrWaters> Anyone here FAIL or RESPOND to kineret?

swimmer> If Enbrel is maybe still working, but I switch because of the shortage, will I have a better success rate

InsightHostKJ> katie..personally i think it will be a LONG time before mtx is a thing of the past....we still have no comparable track records with the new drugs. We KNOW the long term effects of mtx

swimmer> and can you go right from one to another

DrWaters> i AGREE WITH kj.

sandrich> ?

Katie> I know KJ..but I am just wishing, although with the addition of Nexium to my cocktail...my stomach is doing so much better

DrWaters> What disease is it that causes me to keep hitting the cap locks key?

Katie> lol

InsightHostKJ> Capititis?

Texas> Dyslexic Fingers

DrWaters> WHen prilosec goes generic switch to that.

InsightHostKJ> sand go ahead

sandrich> are you near NC so i can come to your office? I'm not kidding!

Katie> ok...so far internist give me samples..but I here samples are going to be athing of the past too

su-boo> ?

DrWaters> No, I'm in Florida. My favorite patient just moved back to NC. Maybe I'll move, too.

InsightHostKJ> swimmer ask yours again, i think it got missed

swimmer> LOL...

sandrich> i will pay your flight!!!!!!!lol

DrWaters> What do you patients think of drug marketing?

swimmer> OK...If Enbrel is maybe still working for me and I switch to a new drug because of the shortage problems, will my success rate be higher, and can I switch from Enbrel to another right away

Katie> FALSE HOPES

bedipped> lol Dr. Waters you don't want to know

swimmer> I have a strong opinion on that...obviously Enbrel has been marketed too good

DrWaters> Yes, Kineret working well in switchovers.

Bren> Too expensive

InsightHostKJ> you mean do we beleive all these meds will have us ice ckating and playing baseball tomorrow

DrWaters> Baseball.

InsightHostKJ> i did pitch a few today...must be working! lol

Texas> I am disgusted with their 2-3 page magazine advertisements.. They can take that money and lower the prices a bit. Have seen 20-50%% increases in my med costse

swimmer> I am still waiting to do that Celebrex Yoga

DrWaters> I didn't mean that. I meant all the MONEY spent on marketing that raises the cost.

InsightHostKJ> LOL Swimmer

Lucy> lol

Katie> Most of them trivialize what RA is really all about....LOL Swimmer me..too..lets do a commercial together..betcha we won't sell any drugs

Melanie> Should be put in programs to assist patients. the market is there - don't need all the smoozing.

swimmer> I could do without my enbrel coffee cup and cooler

InsightHostKJ> i am still seeing enbrel commercials and they dont even have any..what a waste

Lucy> lmao

bedipped> hey at least you got one, Swim...lol

InsightHostKJ> lol swimmer....or my kineret starter kit...

DrWaters> Are any of you in the Radius registry program.

Lucy> RA is a big money business

Lucy> what's that

swimmer> Plus, once you hear all the side effects on those commercials...why would you want to take any

Katie> and that Enbrel sight...what a bunch of nothing...what is it called Access or something...baby info..

InsightHostKJ> whats that?

InsightHostKJ> lol katie.....i agree

DrWaters> Immunex pays us Rheum's to collect data on RA patients.

InsightHostKJ> and i was worried it would be competition for us

Texas> ???Radius Registry?

DrWaters> Yes.

Lucy> I know the doctors are in on the bucks

InsightHostKJ> dont they have enough info????

DrWaters> I would think so.

bedipped> was wondering what more they needed

Melanie> Data only, Dr. W? or specific info on patients?

DrWaters>vThat's my point.

swimmer> like what kind of data?

Gary> isnt that private?

InsightHostKJ> and how much are they paying you? LOL

swimmer> can you let them know how mad we are about the shortage?

Katie> I actually wrote Diane Sawyer after she interviewed Kathleen Turner plugging the Immunex site Arthritis Access and told her if she wanted to really learn about enbrel to come to your site KJ

DrWaters> Stuff like pain 1 to 10. What you can or can't do. Same questions at every follow-up visit. It's BS.

Texas> I am an Enbrel Casualty..

Katie> Texas share your story with Dr. Waters..I would like his opinion

Marcia> so am i

InsightHostKJ> lol katie....they won't....we aren't a major big bucks company

su-boo> me too

DrWaters> The patients have to sign a 5 page long consent form.

InsightHostKJ> what a waste dr w

su-boo> I didn,t consent to no use of my arms

DrWaters> Who's Kathleen Turner?

Marcia> lol

su-boo> actress

Katie> now doing a play on Broadway

Katie> she has RA

InsightHostKJ> The AF hounded her for years to come out and she wouldnt..Immunex pays her and she does

su-boo> not too famous lol

swimmer> in the nude for one part

InsightHostKJ> IRKs me to no end

DrWaters> Oh, I know now. She has RA?

InsightHostKJ> i bet SHE is getting her enbrel thru the shortage!

InsightHostKJ> yup Dr W

Texas> I was on Enbrel less than 6 months went into heart failure after a week in hosp was recovering some of ejection fraction back up to 60 %% then went back on Enbrel and failure again Ejection fraction back to 20%% then finally after months of coreg back to 33 %%

Katie> I wanted to jump through the TV and shake them both....they were acting like the Immunex site was a non for profit fabulous site to help people understand RA....Maddening

su-boo> BS

Texas> All the docs seem to agree was Enbrel

Marcia> BS

Marcia> ?

DrWaters> I hope your doctor submitted a report to FDA and Immunex.

swimmer> ?

InsightHostKJ> go ahead marcia

Marcia> what is BS?

su-boo> lol

Texas> I had to fire that Doc. He didn't know much about much.

su-boo> LOL

DrWaters> Bad synovitis.

Marcia> ohhhhhhhhhhhhhhhhhhhhhhhhhhhhhhhhh, i get it now...lmao

InsightHostKJ> ROFL

su-boo> ahaha

Marcia> gggeeeeeeeezzzzzzzzzz

Katie> LOL....lots of that...specially with news shows....LOL

Marcia> duh

su-boo> marcie marcie lol

su-boo> mardia

su-boo> lol

DrWaters> Time for one last question.

InsightHostKJ> who wants to ask it?

td> ?

InsightHostKJ> go TD

td> Do you reccomend Fosinex?

DrWaters> Never heard of it.

td> or anything else with pred?

su-boo> ?

DrWaters> A lot of patients need a little pred.

Bren> *I think td means Fosomax*

td> yep

DrWaters> Yes, Fosamax or Actonel for non-child bearing age females on Pred.

InsightHostKJ> Dr Waters..thank so much!
