I don't know about you, but I find it hard to keep track of all my medications and what I take them for. A handful for the arthritis, a couple for the stomach, one for water retention, the list goes on and on. When I see a new doctor who wants to know what I take, I always forget a couple if I don't write them down.

We've tried to make that a bit easier. We've designed a Medication Records that you can fill out online, then print out as many copies as you need. You should keep one at home, give one to a friend or family member in case of an emergency and a few more on hand for doctor appointments.

We debated on making this information savable and password protected. But even with password protection, it could be viewed by others. And we just didn't think that was a good thing. The information you put on the form is on your screen only. Once you print it out and leave the page it is gone. No one but you will ever see it.

To fill it out, simply click with your cursor in any of the fields to start typing. You can use your tab key to move to the next field.
