Medical History

You know that looong history sheet you have to fill out when you see a new doctor? The one that wants to know if you or ANYONE in your family has had any of the millions (well it seems like anyways) of diseases they list?

Well we made one for you to fill out at home. Some physicians may take ours instead of making you fill out on there, but if they don't it still makes things easier if you already know all the answers. It's also handy to hand to the folks in the emergency room, saves them and you some time. 

Again we've created two different versions of this form. One can be filled out online, just fill in the boxes and print. The other can be printed and filled out by hand.
