How do I ask and get my medial records?  When I asked the doctor for copies of the  test and what she did, she looked mad and said they would have to make copys or something and walk out.  Haven't seen or heard from her since.  Do I have a right to all of her records or just the test?  I think the cancer is in the bone and she don't want me to hear it from her. 

I was going to ask for a copy of everything tomorrow at the center,  so was wondering how to bring it up and what they have to give me.  If I can't trust them maybe I can read the truth.   I'm fearing the worse, they do more harm by not telling you.  oh well things will be better tomorrow  or maybe the next day.

Below is a sample that I got off of my doctor's website to request medical records.  It is worse to be kept in the dark- hope you get to the bottom of this soon!

___________________________________________________________________________You have a right to your records.  Your doctor has a right to charge you for copying these.   Each state has their own fee that is allowed.  Google your state so you know what is allowed as there are some office's that either do not know the real fee or try to get more.  There is also a time limit that the office is allowed to get this to you.  You'll have to sign a release and I'm not sure of the time that may be a state thing too, however I'm thinking it should be within 2 days.Hi Rusty,
 
Yes, you are entitled to your records.  I request all copies; at first the hospital lab didn't want to release the information, but, now they know me and I fill out their request form every time.  The oncologist always had copies ready for me and readily shared all test results,  reports, etc.  The form that wanttobe included in her post is a very good one and it is all you should need.  Good luck and keep active in your care...hiking_gal
Wilma - it just has to be in writing.  You contact the facility, ask for their medical release form, fill it out, and get it to them pronto.  Or do the form above works too.  The charges are usually minimal, you haven't been with her that long, one surgery?  I hope this sign of a little P&V on your part means you are feeling better.  Eat.  Eating is good for you.  Take care ~~ CathyThank you, I alway get what I need here.   I'm going to print a copy of the form and take it with me tomorrow. 
Thanks RustyThey should be able to produce the form for you to sign but since it is here print it out.
