Dear Patient:

On behalf of eStyle Research, a Medical Research Firm, we would like to personally invite you to participate in our upcoming research undertaking.  We are currently conducting a study in hopes of better understanding the wide range of patient experiences with the drug Forteo, a treatment for severe osteoporosis.

We are interested in speaking with: (a) patients who are currently on Forteo, (b) patients who completed the full course of Forteo, and (c) patients who prematurely discontinued Forteo for reasons not related to safety/side effects.  Telephone interviews are 1 Hour in length and offer a 0 incentive to thank you for your time.

By sharing your story with us, you can help us make the Forteo experience better for patients all around the country. We understand that your privacy is very important to you, and your personal information will be kept in the strictest of confidence.  Your story will be added to the stories of other patients like yourself.  You will not be singled out, and no one will sell you anything.  The information you share will not be given to your doctor.

It is very important to our research that you participate in this study.  As we have reached a time sensitive period, we would appreciate a response at your earliest convenience.  To sign up for this study, please visit our website link at www.rx.estyle.biz.  Alternatively you can call us toll free at 1-877-685-2239.  Should this number not be available in your area, please call our New York Medical Division at 347-222-1113.

Thank you in advance for your taking the time to review our invitation and your prompt response to our urgent request.

Best regards,

I just may be interested in this endevor of yours. I was diagnosed with severe osteoperosis by my Primary Dr. not knowing what it was nor why would he be testing me for it. Come to find out the reson he wanted me tested was because he knew I had COPD, and he was concerned about the inhalers I was on for many years. Sure enough after the test, I was in his office and he confirmed to me that I had severe Oasteoperosis, and it was his opinion that it possibly be from the inhalers I was useing for yeras. The worst one was the Advair 500-50. When my Asthma Dr put me on that medication I was never told ANYTHING about the possibility it would cause me to have osteoperosis. I have been takeing the Forteo imjections for about 8 monts now, and I have been expeincing musel spasms, and my bones have hurt reguarly. If you would like to set up the interview you were speaking of my email is clontzd@bellsouth.net. What makes it worse for me is Iam also a Below The Knee amputee on my left side due to a car accident, and just last week had to have my knee on my good leg operated on to repair a torn miniscus and they also had to clean out some other stuff. Iam now on the mend but not walking vey good. I just had the surgery last Wed. Email me and let me know if you think I would be a proper candidate for this study.

Thank you
Don
