Hi everyone,

I am a freelance translator/linguistic specialist in Vancouver, British Columbia, Canada.

I am currently working on a project for a company that pilot-tests patient questionnaires to make sure the questions are worded correctly and easily understood before they are finalized and ready to be used in clinical trials for AS.

What I am collecting are responses regarding the wording of questions in a survey destined for AS patients, not anyone's personal answers to those questions.  So this is a proofreading project only.

The only information I gather with each questionnaire is your first name, your age and your gender.  And I don't divulge anyone's email address to anyone, under any circumstances.

Here are 3 sample questions from the current questionnaire dealing with Ankylosing Spondylitis:

When did you first start experiencing the symptoms of ankylosing spondylitis?  (Don’t Remember -or- Month/Year)

When did your Rheumatologist initially diagnose you with ankylosing spondylitis?   (Don’t Remember, -or- Month/Year)

How many episodes of Iritis/Uveitis have you had in the PAST YEAR?


What I ask is that you read questions like these, and answer the following sets of proofreading questions for each one:

 1. Do you understand what this means?
 2. Do you think the response options are appropriate? (if response options
     are given, for example "a little, somewhat, quite a bit, extreme")
 3. What does this mean to you, in your own words?
 4. Suggested changes, if any:


Requirements:

You must have been diagnosed with Ankylosing Spondylitis
Your mother tongue must be Canadian English
You must have a working email address
You will need Microsoft Word or a program that handles .doc files to answer the proofreading questions
You may need Adobe PDF Reader (free download) to read the questionnaire


I will be paying each participant Cdn for your participation.

I need 5 (five) participants, as soon as possible. Preferably 3 men and 2 women.
