MTX & Folic Acid Question | Arthritis Information

When are you suppose to take the folic acid for when you take the MTX?

Are you to take it some time before your MTX dose, or a little while after the MTX dose, or the same time as you take the MTX dose?

Oh and I am on 7.5mg of MTX. I misunderstood my RD I take my folic acid every morning and take with my mtx also. No issues.  I thought someone said once that they skip folic the day they take mtx, but may be wrong.   Hope the mtx works for you.   Hopefully you won't have any side effects.  I haven't had any.My RD said take the folic acid everyday EXCEPT the day you take the mtx. I guess it could make the mtx less effective if you take it at the same time.
Good luck Joonie. I hope you keep feeling better and better! Thanks for the replies everyone!I take three folic acid tablets every day, including MTX day.Hi Joonie, never on the day of MTX, I have been told that by a few different rd's and my GP, the folic acid affects the MTX and it does not work as well if you take them within 12 hours of each other. I take my folic in the morning, MTX before I go to bed.....Leucovorin 8 hrs after MTXI cannot remember  frain bog.  But I do remember reading and agree with leila that you aren't suppose to take it the day you take your MTX if its weekly like mine was, but definitely call you doctor and leave a message for a ruling.  LOLI always took it the same day too. Always at the same time.....everyday. I didn't take it at all for at least 3 years....my doctors said if I didn't have any sort of side effects that bothered me not to worry about taking it unless I just felt the need.....and I didn't. When I started having the tingling stuff in my legs we started it to see if would help; but it didn't. I still took it for I guess two more years.

I don't take either now....but I've got plenty of MTX experience; or at least I feel like I do.

I was told to take mine daily as well.  Hmmm...I wonder why doctors disagree????

I take mine daily as well. This is a little confusing.

I started off by taking Wed and Fri, followed by MTX Sat.

The new Rd wants it taken every day without an explanation.
I only notice when I had the new script and looked at the directions.

What would the bloods be showing for the change I wonder.

Any folate regimen better than none

In reply [3], Whittle and Hughes say the issue raised by Bamji—that prescribing habits for folic-acid supplementation differ widely among rheumatologists—was discussed by them in their review and "reflects the lack of sufficient data to make an evidence-based judgment on the optimum dose and timing of folic-acid supplements.

"Given our incomplete understanding of the mechanisms of either the action or the side effects of methotrexate, it is difficult to predict the optimum timing of a once-weekly folate supplement from a mechanistic perspective," they note. "One might argue that providing a supplementary vitamin during the period of effective deficiency (the day after treatment) may have a greater effect than at other times during the week. Clearly, further prospective clinical trials are required to answer these questions."

Regardless of the regimen, supplementation with folic acid is likely to improve MTX continuation rates.

They continue: "It was the intention of our review . . . to provoke debate and highlight deficiencies in the current evidence base. Regardless of the regimen, supplementation with folic acid is likely to improve MTX continuation rates."

This is highlighted by a recent retrospective study of 1022 RA patients treated with MTX, which found that 67% of those receiving supplementary folate were still on methotrexate at 5 years compared with 31% of those who were not taking folate (p<0.001), they note [4].

"We are pleased that the publication of such a review has contributed to the practical management of RA," they conclude.

I take 1 mg daily as prescribed- even on MTX day.  Also took an extra one when I took an extra MTX by accident........had heard Folic Acid negates MTX.
Thanks for the replies everyone!

Ok... my bottle says take once daily. It is 1mg pills.

So... I am guessing I take it one the day I take MTX too.
I stopped getting my Folic Acid via Rx because I was being gouged pricewise.  I now go to Walmart and buy Folic Acid (it is cheap) and figured out what the proper dosage is.  I pocket the savings!  My RA dr asked about it last time I was there because I hadn't renewed the Rx and he was fine with it.
