I thought these might be useful to some.  Other members were kind enough to share with me and I just print them out for use in a pain diary.

This can sure help when you are trying to convey to your RD what you are going through and to track your progress, especially when starting a new drug.

Thanks waddie! Now I have some more things to add to the one I made up for myself. To help me keep track of how this Remicade increase has improved my symptoms. Thanks Waddie, just what I need!  I shall print some up and definitely use them.For the newbies!Waddie, I use a combination of the two forms.  Good for the newbies.  LindyI could see where those pages could be helpful...esp for newcomers
