Does anyone know the name of the form you fill out at each visit asking which joints hurt, activities affected, etc? I am trying to find one online so that I can fill it out accurately (not rushed!) in the week prior to my rheumie visit.

Most doctors use some format of the HAQ (health assessment questionaire). There are different formats but the all the short forms are pretty close. You can always call your dr's office and have them fax or mail you one.
